Met School Extended Day Off Campus Request Form

Providers: Please use this form to send in a request for any off-campus trips that your program may include. This request is subject to the Director of the Met Community School. If approved, the Director will send out a confirmation email and then put in a request with the Met's Transportation Department. Only the Director can submit transportation requests for Extended Day Program Trips. All trip requests need to be submitted 2 weeks prior to departure date. It is at the discretion of the Director to deny any requests that are not aligned with program goals and objectives.

All Extended Day off-campus trips need to happen within the allotted time of your program from 3:30-5:00 PM. All students NEED to be back on the Met Public Street Campus at 5:00 PM to take their transportation back home. Many students take Met busses, public transportation and/or get picked up by their parents/guardians at this time.
