For the past 41 years, the Annual Volunteer Recognition and Awards Dinner has served as an opportunity to thank the most dedicated community members in the city. It is with great honor that we begin organizing the dinner, in partnership with the City of Hayward, to continue to pay tribute to so many selfless volunteers.

Please join us, as we honor these volunteers at the 42nd Annual Volunteer Recognition and Awards Dinner on October 8, 2019, at St. Rose Hospital’s Grand White Tent.

We invite your agency to participate and to select a volunteer to be recognized as part of the “Hearts of Hayward” awards at this remarkable event. Below is a nomination form that must be returned to us no later than May 15, 2019. After your nomination has been received, you and your agency will be contacted by a member of the Volunteer Dinner Committee who will provide further information of the event.

Once the award winners are chosen, the volunteers’ contributions to the organization will be highlighted in an event program and ceremony. We ask award winners please attend the event as complimentary guests of the Volunteer Dinner Committee; up to two guests will be complimentary for multi-winner awards, per agency.

In addition and optional to selecting a volunteer from your own agency, you may nominate for the John N. Pappas Humanitarian award. The Pappas Humanitarian Award is presented every year to a community volunteer who exemplifies community spirit and ownership. If you know of a deserving individual that has excelled in his or her field, please offer your suggestion on the attached nomination form.

  • Please submit two to three paragraphs articulating your reasons for nominating the individual and what work they do for your agency, focusing on their special talents as well as pertinent biography (300 words max).

    We encourage you to include relevant supplemental information about the nominee, such as a current resume with work history and additional materials such as letters of recommendation or commendation from employers, co-workers, or clients.
  • Please upload a .jpeg or .png file format. Photos should be from waist up (no extreme close-ups) and not blurry.
    The file size should be at least 50KB in size, not to exceed (3MB) and at least 500x700 pixels.

    For best results, use larger images with higher resolution for greater clarity. Small pictures will reproduce poorly. If you are scanning in a photo, please use the highest resolution settings (300 dpi).
