Fairfield Department of Health is asking for comments and opinions on our Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). These plans are required for Fairfield Department of Health to become an accredited agency. The Community Health Assessment is a snapshot of the health of the community. The Community Health Improvement Plan is the priorities that the community- at- large has chosen from the Community Health Assessment to address in order to improve public health.
