Parent Volunteer Time Card
The purpose of this form is for the East Wake Academy Administration and the School Board to review volunteer time. This is in agreement with the EWA Handbook and enrollment application. Each family is responsible for volunteering a minimum of four hours per month to assist the school. IF the family is unable to meet this responsibility, they can donate the equivalent of hourly wages in lieu of volunteer hours. This will help with classroom expenditures.

  • Please allocate your donations and send your receipts to the attention of Amanda Boykin, Elementary Receptionist.

    Instructions:
    1. Record your volunteered hours and/or donations.
    2. Complete and Submit the form below by the end of each month. This information
    will be sent to Mrs. Amanda Boykin.
