VACCINATION REQUIREMENTS!

We strive to maintain a safe & clean environment for everyone, while stressing the importance of AVSAB recommended positive puppy socialization between the age of 8-16 weeks as well as a behavior modification program for more serious concerns. For this reason, we do REQUIRE the following for all dogs in classes:

GROUP CLASSES:
Dogs up to 16 weeks: age-appropriate veterinarian administered vaccinations for distemper, leptospirosis(12 & 16 weeks), rabies(12 weeks), and bordetella. Negative fecal and de-worming. Vx history provided as the puppy receives their series of vaccinations.

Dogs 17 weeks – adult: current on veterinarian administered distemper, leptospirosis, rabies, and bordetella. Negative fecal and de-worming or monthly preventative.

PRIVATE CLASSES:
Dogs up to 16 weeks: age-appropriate veterinarian administered vaccinations for distemper, leptospirosis(12 & 16 weeks), rabies(12 weeks), and bordetella. Vx history provided as the puppy receives their series of vaccinations.

Martingales, gentle leaders, and easy walk harnesses are accepted.

Directions: Turn into "Bunk And Biscuit"(NOT Painted Gates) and follow the road around till you see the "DAYCARE" sign above the door, enter the door. Immediately turn to your right and enter the door on your left. Please wait here if there is a class already running inside the fence. You are welcome to watch the class finishing up if there is one, however please wait patiently and QUIETLY so as to not disrupt the class.
Payment: Payment can be brought to class the first day via cash, check, or card. You can also pay in advance by calling Bunk and Biscuit at 269-684-2865. Please make check's payable to Bunk and Biscuit.

*WEEKEND CLASSES*
I attend many continuing education classes that are only held on weekends. There will be no classes those days. Any weekend classes that are effected will be extended by a week. As an added bonus, I do allow you to come to any weekday class of that same week at no extra charge. To know ahead of time, take a look at the class calendar and it will usually be posted 2 months in advance, as well as talked about 2 weeks prior in each class.*

WAIVER, ASSUMPTION OF RISK AND AGREEMENT TO HOLD HARMLESS
I understand that my attendance at a training class is not without risk to myself, members of my family or guests who may attend, or my dog, because some of the dogs to which we will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care or otherwise, and because accidents may happen in the course of moving about which may cause physical injury.

I hereby waive and release PAW IN HAND TRAINING , MELISSA SIEDENSTRANG, BUNK AND BISCUIT, their landlord, employees, and agents from any and all liability of any nature, for injury or damage that I or my dog or members of my family or my guests may suffer, including specifically but not limited to any injury or damage resulting from the action or inaction of any dog or its owner or handler, or from the action or inaction of PAW IN HAND TRAINING , MELISSA SIEDENSTRANG, BUNK AND BISCUIT, their landlord, or their employees or agents, whether arising from negligence or any other reason or cause whatsoever, and I expressly assume the risk of any such damage or injury while attending any training session or while on the training grounds or the surrounding area thereto.

In consideration of and as inducement to the acceptance of my application for membership in this or any PAW IN HAND TRAINING dog training class, I hereby agree to indenify and hold harmless PAW IN HAND TRAINING , MELISSA SIEDENSTRANG,BUNK AND BISCUIT, their landlord, or their employees and agents from any and all claims by me or claims by any member of my family or any other person accompanying me or my dog to any training session or function of PAW IN HAND TRAINING, or while on the grounds or the surrounding area thereto as a result of any action or inaction by any dog, including my own, or its owner or handler or from the action or inaction of PAW IN HAND TRAINING , MELISSA SIEDENSTRANG, BUNK AND BISCUIT, their landlord, or their employees or agents whether arising from negligence or any other reason or cause whatsoever.

I further undertake and agree to indemnify and hold harmless PAW IN HAND TRAINING , MELISSA SIEDENSTRANG, BUNK AND BISCUIT, their landlord, employees and agents from all third party claims, costs, actions or causes of action which may arise at any time as a consequence of any acts or omissions of myself, my dog, my family members, or any other person accompanying me or my dog.

I further agree and understand that there is never a guaranteed end set of behaviors as a result of any amount training, especially in aggression cases. I understand that PAW IN HAND TRAINING , MELISSA SIEDENSTRANG, BUNK AND BISCUIT, will make every effort to ensure proper training of my dog but will in no way guarantee the behavior of any animal at the end of their training.

I further agree that there are no refunds after payment is received. Any refunds due to extenuating circumstances (pet death) will be at the sole discretion of the trainer.

I acknowledge that I have read and understand the terms and provisions of this WAIVER, ASSUMPTION OF RISK AND AGREEMENT TO HOLD HARMLESS and that I am signing
and delivering the same freely and voluntarily and unconditionally.

This form will 'hold' your place in class, however the group classes are in high demand and we limit each class to 6 dogs. If something comes up, and you can't attend the class you signed up for, please kindly let me know so we can offer your spot to another puppy that would like to start class sooner.
