Please read this form carefully and be aware that in participating in activities within the Teen Center you will be waiving and all claims for any losses or damages and injuries you might sustain arising out of your participation at the ‘For the Cause’ Teen Center facility and grounds.

By signing up and participating in activities and trips within the Teen Center that may occur, you will be expressly assuming all liability. I also agree to INDEMNIFY AND HOLD ‘For the Cause,’ Inc, HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees, brought as result of my involvement in the activities of the Teen Center and to reimburse them for any such expenses incurred.

I recognize and acknowledge that there are certain risks of participation, and I voluntarily agree to assume the full risk of any and all injuries, damages, losses or causes of action whatsoever I may have (or accrue to) as a result of participating in all these programs/activities against the ‘For the Cause’ Teen Center, including their officials, agents, volunteers and employees.

I have read and fully understand the above and understand the risks and waiver and release of all claims so set forth above.

By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal.
