Competitive Edge Fit strives to provide the safest and most advanced speed and agility training program possible. However, certain risks are inherent in sports training and cannot be eliminated without destroying the character and nature of the activities. Every attempt shall be made to minimize the existing risks to your minor child through proper equipment, safe facilities, sound safety practices, and up-to-date training of our sports medicine professionals. However, I realize that these risks cannot be eliminated completely. These (print parent or legal guardian name) inherent risks can be the cause of injury, illness, or, in extreme cases, permanent trauma, disability, or death.

I understand that participating in this sports camp is potentially hazardous for my minor child, and that I should not allow my minor child participate unless he or she is medically able and properly trained. I assume full and complete responsibility for any injury or accident that may occur while my minor child is participating in the camp or related event(s), or while my minor child is on the premises of recreational parks or school facilities. I am aware of and assume all risks associated with my minor child’s participation in this camp, including but not limited to falls, contact with other participants, sprains, strains, or other injuries associated with this type of activity.

I hereby waive, release and forever discharge Competitive Edge Fit, the Host School or Recreational Park, trainers, event organizers, sponsors, promoters, and each of their agents or representatives, and all other persons associated with the Speed and Agility Program from any and all liabilities, claims, actions, or damages that I or my minor child may have or acquire against the above-mentioned parties arising out of or in any way connected with my participation or my minor child’s participation in the camp.

I further voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me or my minor child as a result of being engaged in the camp or any camp activities.
I understand that this waiver includes any claims, whether caused by negligence, the action or inaction of any of the above parties, or otherwise. I further attest that to my minor child is physically fit and has been examined by a physician to participate in the Speed and Agility Training program. I give permission for Competitive Edge Fit to provide immediate and reasonable emergency care to my minor child should it be required. I understand that Competitive Edge Fit and the Host School or Park will attempt to notify the Emergency Contact listed on the camp registration form as appropriate before providing any such emergency care.
I certify that I am at least eighteen (18) years of age and fully competent, and that I am the parent or legal guardian of Participant. I further certify that I have carefully read understood the terms and conditions stated herein. I accept these terms and conditions and acknowledge that this agreement shall be effective and binding upon myself, my heirs, assigns, personal representative(s) and estate (as well as those of my minor child) and all members of my family, including minor children, from this date forward.
