This form comes in various formats. It can be shown with all fields (boxes) such as name, address and URL. It can also request information via checkboxes such as shown on my Custom Order page. Each field has the option of being a requirement or not prior to submission. In addition, you also have the option of having the form displayed in a separate window like this one, or you may have a form created to embed as a post. (Please refer to Custom Orders to see how the form displays.) The choice is yours.
You may also elect to require a Captcha Image displayed prior to submission to decrease spam. You will not see that demonstration in this sample, but I would be more than happy to include it for you if you so desire.
I am sure you will agree, this is a really cool form.
