Effective January 1, 1991 the Illinois Uniform Conviction Information Act allows ARC Community Support Systems to obtain criminal conviction information involving consumer abuse from employees or applicants. If you have been convicted of a criminal offense involving a consumer, list the conviction(s) below. You are not required to disclose sealed or expunged records of convictions. Listing a conviction does not automatically disqualify you from consideration.
