NOTE: This is NOT for regular alteration & mending work. Although, you are welcome to submit this form if you alteration/refashion is a unique request.

One consult fee PER request. If you are needing multiple items made, you will need to submit multiple forms, incurring consult fees for each.

Prior to any estimate being provided you first need to submit this form so I can determine if it is a project that I will be able to take on.
A non-refundable $60 consult fee will be required. (Organizations have a $150 consult fee). If you sign your contract within 7 days, $50 will be applied to your order with your final payment.

NOTE: No response will be given to inquiries with insufficient information.

Once your request has been reviewed, you will receive an email confirming receipt and determination of whether your request can be done and within your time frame. At this point you will be requested to submit your consult fee in order to move forward to receive your estimate.
