Copy of picture identification cards for coaches and players, including all guest players or club pass players, issued by the team’s Federation Organization Member (USYS, AYSO, US Club, etc) All cards must be from the same organization member. There can be no mixing of cards.

OHIO NORTH TEAMS (read below):
If you do not have any changes to your roster from your regular league play, we do not need a copy of your cards.

If you have guest players, we need copies of their cards.

If you have a coach not on your tournament roster but is club carded within your club and will be coaching, we need a copy of their card.

Every team must submit a completed compliance form to be eligible to participate. You can find the form on the Bothwell Cup website. All requirements must be met per the guidelines of the State Law. Please refer to Player Safety for the requirements for coaches, parents, and players.

Certificates must be uploaded for all rostered coaches or managers. Ohio North Coaches: If your concussion certificate is uploaded in the Affinity or Stack Sports Systems, you do not need to upload it here.

Teams outside of Ohio North jurisdiction must provide proof of permission to travel
