By filling out this registration form, I certify that my dependent is physically fit to participate in the Fair Oaks 9th Annual Spring Break Basketball Camp. I further state that I will not hold Swiss Sports, Basketball Anonymous, Basketball Phonics or any staff liable for preexisting illness or injuries as a direct or indirect result of participation in camp activities. I give permission for camp officials to seek medical assistance as deemed necessary.
