All Entrants must sign the following authorization, if Entrant is under the age of 18, a parent or guardian must sign:

Entrant waives any and all demands, suits, actions or claims of any kind whatsoever he/she has ever had or will have in the future against Whitemarsh Township, including its officials, agents, volunteers, and any person, firm, corporation or other entity employed by the Township, either directly or by independent contract, for any and all damages, claims, costs or charges of any kind suffered by entrant as a result, either direct or indirect, of entrant's participation in the 4th of July Parade. Furthermore, entrant shall indemnify and hold harmless Whitemarsh Township, including its officials, agents, volunteers, and any person, firm, corporation or other entity employed by the Township, against all claims, suits, actions, costs and demands of any kind made against Whitemarsh Township, including its officials, agents, volunteers, and any person, firm, corporation or other entity employed by the Township, as a result of entrant's participation in the 4th of July Parade. I hereby state that I am authorized to execute this entry form on behalf of the entrant and that the information provided by the entrant or me is true and correct. I acknowledge and agree that my image(s) and/or the image(s) of the entrant, as well as the sound produced by the entrant, if any, may be photographed, videotaped, and/or recorded during participation in the Whitemarsh Township 4th of July Parade, and, in exchange for being permitted to participate in the parade, I further affirmatively grant permission to Whitemarsh Township, including its officials, agents, volunteers and any person, firm, corporation or other entity employed by the Township, either directly or by independent contract, to use those images and/or sounds for purposes of publicity, solicitation of sponsorship, broadcast or cablecast on television or radio, display on the internet, or for any other purpose, now and in the future, forever, by Whitemarsh Township, including its officials, agents, volunteers and any person, firm, corporation or other entity employed by the Township, either directly or by independent contract, without any notice or further compensation to me, and I waive any rights to said image(s) or sound(s) on behalf of myself and/or the entrant, now and forever.
