Thank you for your interest in vending at MadCity Bazaar! Please complete this form to apply to be a vendor for the 2015 MCB Season.

FAQs
- You may choose from 1-22 markets.
- Hours are 10a-4p (except 6/21, 6/28, 7/5 & 9/27 will be 10a-6p).
- Booth Size 10x10
- You may share booth space with another vendor.
- Payment plans can be arranged on an individual basis.
- Our events will run rain or shine.
- Vendor fees are non-refundable.
- You will be required to hold a WI State Seller's Permit.
- General liability insurance will be required for each booth.

You will receive a confirmation email that your submission has been received. Once our team has completed the jury process you will receive a second email with your acceptance status. If you are chosen as a vendor you will receive an email with your vendor contract and you will be invoiced for your vendor fees.

  • If you do not have a WI Sellers Permit number please note if your sellers permit is pending or applied for.
  • If you do not currently carry general liability insurance at this time please write NONE.
  • If you do not have a booth image please "set the stage" and take a photo to give us an idea of your style and presentation. We will not hold you to an exact replica at our markets.
  • Please check all the boxes that apply to you or All Dates if you plan to vend the entire season. Event set-up will begin at 8a and take down will end two hours after the market closes.
