The Creative Dementia Arts Network annual conference, organised in collaboration with Eminence Grise at
St Hugh's College, St Margaret's Road, Oxford
Main conference, Day 1: Thursday 14th April 2016 10.00 a.m. - 6.30 p.m. and
Master classes, Day 2: Friday 15th April 2016 from 9.45 a.m. - 4.00 p.m.

    Conference administration: Eminence Grise has been appointed to provide conference support and administration so please contact Elizabeth Mills with any queries about booking your place.

    Delegate fees: NB you will save £20 if you book both days!
    There are just four categories of delegate -
    Students and self-funded practitioners or volunteers - £70 for one day or £120 for both days;
    Voluntary sector employees - £120 for one day or £220 for both days;
    Public sector or commercial employees - £150 for one day or £280 for both days;
    People with dementia or carers of people with dementia - £10 for one day, £10 for both days

    Payment: Completion of this form constitutes an agreement to pay to attend the conference; an invoice will be sent to you (or the designated payer)

    Information about accommodation and travel will be posted on CDAN website Limited accommodation is available at St Hugh's College at the rate of £75 per person for bed and breakfast. Please contact the College direct as soon as possible on if you wish to book, quoting reference CDAN16.

    Please note that notice of cancellation must be received by the conference administrator by 14th March in order for you to receive a full refund; by 1st April for 50% refund. No refund will be made thereafter, but you may nominate a substitute to attend in your place.
