    If we bring you on as a volunteer, we may ask you to supply your social security number in order for us to preform a background check, especially if you'll be working with children. Indicate below that you undertstand that this may be necessary and can provide a valid social security number when and if requested.

    3) I will adhere to all American Museum of Ceramic Art policies and procedures.
    4) I will readily accept training and supervision from Museum Staff.
