NASW-FL Miami-Dade Annual Awards Nomination Form
The National Association of Social Workers, Florida Chapter (NASW-FL) established their annual Social Work Awards to honor outstanding professionals and members of the community at the local, state, and national level. Qualified individuals nominated will be sent a request for additional information. Multiple nominations and self-nominations are welcome. The Deadline is January 19, 2018.
  • 1: Must be a professional social worker and NASW-FL member in good ethical standing.

    2: Must be a current NASW-FL member and enrolled in a CSWE-accredited social work program in Florida.

    3: Must be a current NASW-FL member who is affiliated with a CSWE-accredited social work program in Florida as a faculty member, adjunct, or field instructor.

    4: Must not be a social worker or have a degree in social work.

    5: Must be an official currently serving in Florida elected office at the local, state, or federal level.

    6: Agency of the Year; Humanitarian of the Year and Lifetime Achievement Award are Miami-Dade Unit awards. Contact the local chair: miamidade.naswfl@socialworkers.org for more information.

    To ensure the nominee’s eligibility, please fill out this form completely and use clear, concise, descriptive examples of activities and accomplishments. Incomplete submissions may be rejected at the discretion of the Committee. The unit’s Chapter Committee on Nominations and Leadership Identification (CCNLI) selects and approves the award recipient.
