ACKNOWLEDGEMENT

I have read and understand the "Contest Host Agreement," which is a separate document, and agree to the terms of hosting an SFWGA contest. I understand that some contests require a Host Fee (if indicated above), and if awarded a contest assignment, will provide an initial deposit of $1,000 no later than October 30, 2011, and the remaining balance of the Host Fee no later than January 10, 2012. I have discussed the possibility of hosting a contest with my Band Director and School Administration, and all have agreed to the use of the campus for this Event. I have verified the availability of the campus on the School Calendar for each date requested above.
