  • The I/DD Eligibility Determination Process:

    Community Developmental Disability Organizations (CDDOs) are given statutory authority to make eligibility determinations for services. That authority, known as Single Point of Entry, is found in the Developmental Disabilities Reform Act (K.S.A. 39-1805).

    To begin the process, the Eligibility Application must be completed and signed with all required supporting documents. Once received, an HMCDDO Access Specialist will then review the information gathered from the Application Packet. All authorization forms will be sent to the applicable entities to gather all relevant diagnostic records & data. After receiving all of the required data, a review of the information will be completed to make a final determination. You will then be notified via written letter (or email) of your “approval” or “denial” of the Eligibility Determination Phase by the HMCDDO.
  • Required information to determine eligibility for I/DD services:

    Your HMCDDO Application will be considered complete when all items listed below have been received:
    1. Signed application form
    2. Signed release forms
    3. Receipt Notice of Privacy Practices
    4. A psychological evaluation (if applicable)
    5. A recent medical examination
    6. A copy of current health insurance card
    7. If applicable: Letters of Guardianship, or Durable Power of Attorney documents, or Child In Need of Care Journal Entry signed by Judge and Case Plan
