Priority Group Consultant Quarterly Reporting Form
All Priority Group Consultants should submit a quarterly report summarizing their work with committees under their priority group area.
If you have concerns or commendations about any of your committee chairs, please send a separate email to the ALSC President with a copy to the ALSC Executive Director.
Please note that the form cannot be saved. Once information is entered it cannot be edited. All required fields are marked with a red asterisk (*) and must be filled in; screen readers will say the word star.
Once submitted, the form will be emailed to the ALSC President, Vice-President, immediate Past-President, and Executive Director. You will receive an acknowledgement email from the Executive Director along with a copy of your report.
