Adoption Application

In order to be considered for an adoption, you must; be at least 21 years of age, have the consent of all adults in the household, have a valid ID with your current address, landlord’s name and phone number or copy of your lease if renting. Completion of this application is authorization for TSDP to confirm any information contained within and to do a background check for any animal abuse or neglect history. Completion of this application does not constitute any agreement for adoption. If you are approved for adoption you will complete the adoption contract.

If something changes in your life and you were not able to keep this pet, what would you do?
If you have adopted from us before, please give the date and pet's name.
Please list each pet you own and have owned for the last five years in the following format: Name, species and breed, age, if the pet is spayed or neutered, if the pet is up to date with vaccines, and the name of the veterinarian that has seen that pet.

Please list any other information you think may be relevant to the adoption of this pet.
By entering your name you are stating all information is true and accurate. Any information found to be inaccurate will result in all pending and future applications to be denied. This application is authorization for TSDP to investigate all information contained within. This application will remain property of the rescue.
