Instructions:

- Evaluation is an integral part of the grant process that is not only necessary for our records, but will also assist us in project assessments and future grant funding decisions.

- Please note this evaluation form all with receipts for ALL purchases are due within a week following your event. Failure to complete this evaluation and provide invoices or receipts WILL result with no reimbursements and affect your ability to apply for future grant funding.

-Please allow one week after submitting your evaluation form for the reimbursement to be processed into your organization's account.
