  • As the parent or legal guardian of the student named above, I request that my child be enrolled in the First Connections Program.

    1. I understand that my child has been chosen to participate in the First Connections program offered by Northlake Christian School based on the admission criteria of the program. It is my understanding that individualized instruction and a student-centered classroom will be provided for my child in preparation for success in the upcoming school year.

    2. I realize that this program will not be held responsible for immediate entrance into the next grade level since multiple criteria need to be met prior to admittance. I understand that the teacher will closely monitor my child's progress and specifically look for patterns of strengths /weaknesses, self-motivation, learning rate, and home participation.

    3. I agree to work closely with my child to ensure school success. I will also be readily available for any necessary meetings or conferences that may be scheduled during the school year.

    4. I understand that the cost for placement in the First Connections Program is $10,020 for the 2013-2014 school year and is due in accordance with the payment schedule set forth within the NCS Tuition and Fees brochure.
  • Tuition and Fees Policy:

    By submitting this document, we agree and acknowledge that the school must make budgetary decisions based upon a projection of total enrollment, purchasing supplies and materials, contacting with teachers and staff, and deciding operational issues based upon our agreement and and representation to enroll our child for the entire school year. As a result, we agree that we are responsible to the school for tuition for the entire school year regardless of whether our child attends for the entire school year. We agree that, in the event that our child is removed from Northlake Christian School or does not otherwise complete the entire school year for any reason, including but not limited to withdrawal, expulsion, relocation, disciplinary issues, natural disasters, loss of employment, financial issues, health issues or excessive absences, failure to pay tuition in a timely manner, or any other reason, we are still responsible for tuition and fees for the entire school year as set forth in the Tuition and Fees brochure published by the school.
