To Request Quote for A4 Document Scan

I'm Asking for a *
 " Price Quotaton" for my budgeting. 
 "Service Sales Order" . I like to confirm/proceed my Order Request. 
If you select " Service Sales Order " you will receive the Service order in pdf/doc with the followup details. If Price quotation is selected, price breakdown will be quoted.
(1) Enter the Approximate Sheet QTY: *
Estimate your number of sheet to scan & convert.
(2) Which source type you will submit for the work? *
 i'll submit Loose Documents. 
 i'll submit Binded Documents that are stapled , clipped, binded etc ( will charge +S$30 per 500 sheets processing). 
 I'll submit a mixture of Loose Documents and Binded Documents. 
Document litigation for binded Documents: Removing all staples, paperclips, binding tape, folds and etc. from the documents + SGD $30 for 500 Sheets per batch
(3) How to Scan your Sheet? *
 Simplex Scan ; Scan one single side for every sheet. 
 Duplex Scan ; Scan both sides for every sheet. 
(4) What type of Scan? *
 Color Scan (150dpi by default) 
 Black & White Scan (300dpi by default) 
(5) Additional Services:
 Scan to Excel file + SGD $0.26 per page. 
 Higher Scan Resolution: 300dpi color or 600 dpi Black & White + SGD $0.02 per sheet. 
 Free Document indexing. plus sort to 2 different folders. 
Document indexing is the process of naming your scanned documents or sorting your documents in different folder names.
(6) Do you want to Index and Sort your Image files to multiple different folders, more than two (2) folders ? *
 No. Just Free Document Indexing and Free Sorting to two different folders only. 
 Yes, All my Documents, Please Index them and sort into multiple different folders (+ S$0.04 to $0.08 per page depend on QTY) 
 Yes. Only for 104 pages, Please Index them and sort into mulitple different folders (+ S$4) 
 Yes. Only for selected pages, Please Index them and sort into multiple different folders (+ S$0.04 to S$0.08 per page * see below *) 
Price Range for Document Indexing:

indexing 900 pages & above = $0.08 per page.
indexing 105 pages & above = $0.06 per page.
min indexing 104 pages , $4 or at $0.04 per page .
(7) Addtional Special Requests / Enquires:
If you intend to submit a mixture of source , Pls define the QTY for each source type .
(8) How do you intend to submit your Hardcopy Documents? *
 i'll Self- Submit my Documents to destinated address (No deposit required, Final Payment due upon work completion. ) 
 i'll Pay a fee for Document Pickup / Collection and Delivery. (SGD $32 for 30Kg, 2 Stack of 54.8 cm x 42.1 cm x 33.5 cm box. ) 
*Subjected to terms & conditions and only for Address in Singapore Mainland only.
*No Upfront Deposit is for sheet QTY 300 & below
(9) How to Store for your Image files? *
 I'll Pay $8 for image files to burn in a new 4.7GB DVD data format. ( For above 800 QTY images, it's compulsory to be charged with $8 for a new DVD disk .) 
 I'll provide my own storage device, Thumbdrive (For below 800 QTY images)
