Thank you for your commitment to safety and welcome to the ISRI Circle of Safety Excellence™ application!

There are two steps to the Circle of Safety Excellence™ application process: information input and data entry.

Step 1: The application: The application collects data about your company's general demographics, safety management program, transportation, and commitment to continuous improvement and industry outreach. The ISRI Circle of Safety Excellence™ application requires OSHA data for the current reporting year, plus three (3) years of historical employee injury data. If your company does NOT have OSHA data for the current reporting year, plus three (3) years of historical employee injury data, please complete this form to submit your available data.

Step 2: Submission of 4-years of safety data: The data entry section requires OSHA data for the current reporting year, plus three (3) years of historical employee injury data be submitted within 30 days from the date of your initial application. You do not need to complete both steps at the same time, but both MUST be completed for you to become a member of the Circle. If you are currently a member of ISRI's Circle of Safety Excellence™, please submit your safety data for the current reporting year ONLY. NOTE: Previous safety data is not amendable and provided in a read only format.
