Studio | 1004 Models are selected for their unique style, outgoing personalities, involvement in school activities and of course your eagerness to be a part of the Studio |1004 Model Program!
You will need parent permission to model for us, so make sure they are around when you fill out your form. If you are chosen to be a Studio 1004 Model, you will be notified via email with your promotional shoot time and date.

Pre-Photo Shoot

For marketing and promotional purposes, we will need to conduct a photo shoot PRIOR to your actual session. This is a FREE session, limited to one outfit which needs to be done by June 1. Please select a date and time that works for you. (Tuesday-Friday only.)

Since sessions are given on a first come, first serve basis, this will not guarantee that you will get one of your choices, but the sooner you get your permission slip turned in, the better your chances will be. We don't shoot on Monday or Sunday.

Just so you know

After the pre-photo shoot, a consultation date will be scheduled to discuss wardrobe and locations for your senior session. A $125 deposit is required at this time. All of your deposit will be applied to your print purchase.

CANCELLATION/REFUND POLICY: Once a session has been scheduled and a deposit paid you will only be eligible for a refund if you cancel BEFORE August 31st. If for any reason you simply need to RESCHEDULE a session, your deposit is transferable to the new session.

YOUR COMMITMENT:
As a representative of Studio | 1004 your images may NOT be used to promote or represent ANY OTHER LOCAL PORTRAIT STUDIO for 1 year. Exclusions apply to team pictures, dances, and/or professional modeling.
