  • You must have had a consultation done by one of our representatives prior to booking your training program so we can help determine if this is the right option for you and your dog. This also prevents and schedule conflicts.

  • EXPLANATION OF SERVICES

    (a) Dog training, in a group or individual setting -- private lessons by appointment only.

    (b) Boarding of dogs for the purpose of training

  • PLEASE READ THE FOLLOWING INFORMATION CAREFULLY BEFORE SUBMITTING/ELECTRONIC SIGNING

    A. Complete Obedience Dog Training (“CODT”) shall exercise reasonable care to maintain a clean, safe and secure environment for the dog while in the care of CODT. The dog shall be fed properly and regularly; shall be housed in its own quarters; shall be brushed, cleaned and groomed as necessary; and shall not be allowed off of CODT’ premises without the consent of the owner of the dog.

    B. The dog will be trained to perform as shown through a demonstration. Training sessions with the owner shall be included. Owner understands and agrees that if the owner fails to cooperate fully in completing the owner training sessions, CODT cannot and shall not assume or accept responsibility for the behavior or performance of the dog after it has been returned to the owner.

    C. If the dog evidences a medical problem during the time the dog is under the care and control of CODT, CODT shall take such measures as it deems necessary or desirable, without being liable to owner, without limitation, the right to administer medicine or to obtain an evaluation by a veterinarian of CODT’ choice. If, in the opinion of CODT or the veterinarian selected by CODT, the cost of treatment exceeds seventy-five dollars ($75), CODT shall make reasonable attempts to contact the owner, and/or the veterinarian designated by owner above, for further instructions. If owner’s designated veterinarian cannot be contacted, CODT or the veterinarian selected by CODT may, without liability to owner, provide such emergency medical treatment as may be deemed necessary to preserve the health of the dog. Owner shall pay the cost of all treatment provided for in this Section, in addition to other expenses called for in this Agreement prior to the dog being picked up by owner.

    D. If during the training of the dog training is interrupted for any reason that is not the fault of CODT (including the illness of the dog), the owner may, at owner’s option, continue to board the dog with CODT, at its then existing daily boarding rate ($30/day), until training can be continued. The cost of boarding shall be in addition to the training contract price. Owner may pay any balance due on the training contract price and remove the dog from CODT’ premises, in which case the dog may be returned to CODT at a later time for completion of training at no additional cost. If any delay or interruption in training is the fault of, or at the request of CODT, including delays for stress which may show up during training, owner shall not be charged for any such delay or interruption.

    E. CODT agrees that while the dog is on its premises and under its control for boarding or training, CODT will exercise the utmost diligence and care to protect the health and safety of the dog. However, owner agrees that CODT has no liability for loss of or damage to the dog or owner’s other property resulting from fire, disease, illness, virus, escape, theft, death or other unavoidable causes. After the dog has been returned by CODT to the owner, CODT cannot and will not accept any responsibility for the illness, injury, or death of the dog or of any person or animal, or damage to any property, caused by the dog.

    F. If at the completion of the agreed boarding and training time the dog is not picked up by the owner and/or owner fails to make payment in full of all charges, including any medical, veterinarian or boarding charges as provided herein, CODT shall notify owner by certified mail at the address shown above that the dog will be held for an additional ten (10) days following the date of mailing of the notice. At the end of the ten day period, the dog may be sold at public or private sale. The proceeds of such sale shall be first applied to the charges of CODT and the balance, if any, shall be returned to owner at the address shown above. Owner shall be charged for any such additional days following notice at CODT’ then existing daily boarding rate ($30/day).

    Owner represents that owner is the legal owner of the dog; that the dog is free and clear of any encumbrance; that the dog has not been exposed to distemper, rabies or parvo within the past thirty (30) days; that the dog has been inoculated as shown above; that the dog is currently and properly licensed, on flea/tick/heartworm preventative, and that owner has read this agreement in its entirety.
