These documents will not be seen by anyone outside of LIBA staff, and will be held completely private. YOU CAN ENTER THIS CONTEST IN ONE OF TWO WAYS:

A) proof of purchase from a fellow LIBA member for your business – scan or take a picture of an invoice or receipt (amounts and other sensitive information should be blacked out) and attach it to this form. (Must be dated in 2016.) OR

B) Proof of an estimate or quote from a fellow LIBA member for your business – scan or take a picture (amounts and other sensitive information should be blacked out) and attach it to this form. (Must be dated in 2016.)

Check this box if LIBA can publish your testimonial. Your documents will not be released, only your testimonial.

Each vendor relationship counts as one entry. (Not each purchase.) For example, you may purchase from XYZ company every week, but this only counts as one vendor relationship.

You can enter as many times as you like with your various vendors and estimates. This form holds up to 10 entries. Use more than one form if necessary.
