FHR ADOPTION APPLICATION
Fetching Hope Rescue represents an all volunteer canine rescue group whose mission is to save the lives of neglected, abused and/or stray dogs by finding them safe, loving, and permanent homes in New England. Please complete the following application in its entirety. This application is designed to help us assist you in finding the right dog for your family. We will check vet or personal references provided and then have a volunteer contact you for a home visit.
Animals are not short-term commitments. Make sure your lifestyle allows the time, patience and expense this pet will need over the years before completing this application. An adoption fee in the amount of $450 for all dogs, unless specifically marked, is due at time of adoption and this covers previous expenses of the dog- appropriate vaccinations, altering, microchipping, health certificates and transportation fees. We include a dose of heartworm preventative with each dog. Protect your pet!

We request an adoption fee of $450 for all dogs. This pays for the shelter to alter the dog, appropriate vaccinations, microchips, transport fee, and an initial dose of heartworm preventative. Is this acceptable?

By submitting this application, you understand this is the first step of the adoption process. Fetching Hope Rescue reserves the right to refuse adoption to anyone. You certify the information contained within this application is accurate and not misleading in any way. Our organization reserves the right to contact individuals listed on this form for verification.
By signing this application, you understand that we make every effort to ensure every animal is healthy and all records will be passed forward to the adopter, but FHR is not responsible for future vet care. We make no guarantees in regards to health or temperament and our goal is to provide the most knowledge possible about each dog, but you will be adopting your dog “as is.” Due to the fact that most of our adoptable animals are rescued as strays, we cannot guarantee their breed. We list the pets breed/s as the one/s that best matches their appearance and personality.
Every dog arrives microchipped and spayed or neutered, with medical records and the interstate health certificate which will accompany the animal on transport. In order to travel, dogs must be up to date on shots, which include DA2PPv Canine Distemper, Bordetella (kennel cough vaccine,) Rabies (if over 3 months,). Dogs must test negative for heartworm, and must have a fecal test that is negative for worms, coccidia and giardia. They are all given Flea/tick preventative before boarding the PETS truck.
By submitting this application, you understand this is the first step of the adoption process. If approved for adoption for any dog with us, by signing below, you also agree that if the adoption takes place and you are unable to keep your dog for any reason, the dog will be returned to our organization, Fetching Hope Rescue and will not be returned to a shelter or re-homed privately. We will refund adoption fees (minus a $50 processing fee) within the first two weeks, if the dog placed is not a right match for your family. If a return does occur, we ask that you are able to hold the dog until appropriate foster home is found, up to two weeks maximum. Information regarding returns and contact will be provided in official approval letter. This is a legally binding contract.
