The Community Directory is managed by Tuam Community Forum and provides details of community and voluntary organisations, religious groups, education institutions and statutory agencies serving Tuam Co. Galway for the benefit of the public and the organisations themselves.

This form will take about 10 minutes to complete.

There are 4 sections:
1. YOUR ORGANISATION - what you do and how people can contact you
2. ORGANISATION STRUCTURE - how you are organised and supported
3. WHO USES YOUR GROUP - the people you support & your facilities
4. HOW CAN WE HELP YOU - your feedback and training needs

We wish to make the information you provide in Section 1 & 3 available to publicise your organisation and will ask you to agree/not agree to this at the end of this form.

Please note: Section 2 & 4 are for internal use ONLY by Tuam Community Forum and will not be released to the general public.
