I give my permission for their involvement in Eagles Wings Track Club/Minneapolis Believers in Christ activities. I have reviewed the stated expectations for athletes and agree that my child will abide by them. Dismissal from the track program for non-compliance is understood. I grant permission to Eagles Wings Track Club and Minneapolis Believers in Christ for the use of any video images, photographs or other audio/visual reproduction that may be taken of the child for purposes of promotion and public relations. I understand that Eagles Wings/Minneapolis Believers in Christ and its agents will take all reasonable safety precautions at all times during events and activities. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree not to hold Eagles Wings/Minneapolis Believers in Christ its leaders, employees and volunteer staff liable for damages, losses, dis-eases or injuries incurred by the child listed on this form. Should it be necessary for my child to have medical treatment while participating, I hereby give the staff permission to obtain medical services. I give permission to the physician selected to render whatever medical treatment is deemed necessary. I understand that the Eagles Wings Track Club/Minneapolis Believers in Christ has no insurance covering medical treatment or hospital costs; therefore, any cost for treatment shall be my sole responsibility.
