The registration fee includes: NCHN sessions, Disney Institute program, Monday & Tuesday night receptions, 3 breakfasts, 2 lunches, beverage breaks, and conference materials.

"Although Disney Institute is presenting the Disney's Approach to Business Excellence on Wednesday, April 20th, the sponsor of this program, National Cooperative of Health Networks, and not Disney Institute, is responsible for marketing and promoting this program, and for providing and arranging for the venue, facilities and other items for this program. This includes any food and beverage offered at or in connection with this program. National Cooperative of Health Networks is not an agent of Disney Institute or its affiliates, and Disney Institute and its affiliates assume no liability relating to the event or for the acts or omissions of National Cooperative of Health Networks."

    By clicking "Submit" you are agreeing to pay your registration fee. You are also giving permission for your name and contact information to be published in any listing of participants that may be prepared and released by NCHN. In addition, by submitting your registration, you are consenting to allow NCHN to share the Registration Data with the Disney Institute (DI) for DI's use (a) in connection with the Program (including, without limitation, sending participants an email requesting that they complete an optional survey as to the Program), and (b) in sending promotional materials to participants as to DI's products and services (unless participants opt out of receiving such promotional materials). If you want to opt out of receiving such promotional materials from DI please check the box at the bottom of this page.

    Please note: Registrations will not be accepted from individuals associated with corporate organizations or non-profit organizations without registration as a conference sponsor.

    You are also consenting to allow the Renaissance Oklahoma City Convention Center Hotel to provide NCHN your room reservation information. Specifically your name, company name, room rate and arrival and departure dates.

    Cancellation Policy
    The registration deadline is April 4th. If for some unfortunate reason you should be unable to attend the 2016 NCHN Conference after registering, NCHN will refund the registration fee if you meet the following guidelines:

    The 2016 Annual Conference Registration fee, minus a $50 processing & cancellation fee, will be refunded after the conference, if written cancellation is received by Linda Weiss at lweiss@nchn.org by April 11, 2016. Unfortunately, if a cancellation request is received after April 11, 2016, NCHN cannot refund the registration fee. If the fee has not already been paid, it will be due upon receipt of an invoice from NCHN. NCHN is unable to provide a refund after April 11, 2016 because NCHN will be committed to all applicable conference expenses.

    If you should need to cancel after April 11, 2016 and wish to send a substitute, NCHN will switch the registration to the new conference attendee, but cannot guarantee that this person will be included in the conference listing of participants and/or have a printed name tag.     Upon submission, you will be redirected to the Plan Your Trip page. There, you will find a link to PayPal and information about reserving your room. You will also receive a copy of your registration by email.
