GUIDELINES FOR AVAILABILITY CHANGE- READ CAREFULLY
●A reason must be provided as to why you're changing your availability.
●Associates MUST be able to work at least one weekend day. This form will not be submitted if you put that you cannot work Saturday or Sunday.
●If you're submitting a request that will be effective when the schedule for that particular week is already made, you must find someone to cover your shifts.
●Associates that have been recently hired within 30 days or less may not use this form to change their availability.


**HOW TO FILL OUT THE CHART BELOW**
●If you're not available on a specific day, put "X".
●If you're available the whole day, write "all day".
●Your availability should reflect the earliest you can get to the store and the latest you can stay.
●If you must leave at a certain time due to transportation, specify the time.
●If you can stay till close, write "close".
●Make sure you have transportation arrangements- you will not be released until the store is fully recovered if you put that you can close.
●If you're a minor you must leave by 11:00 PM if there is school the following day.
●The earliest Sales Floor associates will get scheduled is 9:45 AM.

Monday: all day
