2. All outcomes were documented in CC as stated in the notes below
3. If I cannot get to this form, I will notify CT and ask CMS to do it or I will review the outcomes with him

EVERY QA CALL OUTCOME IS TO BE DOCUMENTED IN CC VIA THE FOLLOWING
1. Go to CC for Client and modify
- log a note called QA Call and just the summary of this (Related to: Client and the CG)
- CG Preferences (Favorite or Do not sent)
- Matching Criteria (Favorite or Do Not Send)
2. go to CC for CG and modify Matching Criteria and HR Notes
3. Email Care Managers with any updates that are important for Staffing Instructions
