I understand that my agreement with Fit Bodies Forever, Inc., may be canceled at any time with a one-month written notice (at least five days prior to next full billing period) through a Fit Bodies Forever Request for Cancellation Form located and accessible through our website, www.fitbodiesforever.com. Email, texts, voice mail, written letters, verbal in person or any other type of message requests to cancel do not constitute acceptance of any cancellation. Completion of a Fit Bodies Forever Cancellation Form is required and must be signed and dated by member and a Fit Bodies Forever representative. Online Fit Bodies Forever Membership Cancellation Forms must be electronically signed by member and submitted. If online access is not available, member is responsible for obtaining a Fit Bodies Forever Membership Cancellation Form by calling via phone or visiting in person Fit Bodies Forever, Inc. We do not offer refunds for partial months under any circumstances. Monthly payment still remains in effect during the next billing period. Membership will expire thirty days from date of final payment. If a membership is cancelled and then resumed within the same calendar year, a one- time re-enrollment fee of $30 (current fee subject to change) is required. Re-join rate will reflect current monthly membership fees.
I understand that if I reinstate my membership at a later date, my rates will reflect the current monthly dues.
I understand that it is my responsibility to ensure that all billing has stopped, and I will contact Fit Bodies Forever, Inc., immediately if billing has not ceased as it was intended.
