  • Lending Library Agreement

    Provider Eligibility:

    Teacher must be an active Provider with a caseload of no fewer than 5 hours per week.


    Provider must be in good standing with NYTPS Inc., and willing to be an active participant in the lending library program. We expect cooperation to sustain this opportunity for you.

    All providers are expected to complete assessments for the students they serve.

    Terms of Use:

    Once your order is received and processed the requested items will be mailed to the address provided above. If an order was made, please call Stephanie Deshommes at the main office 718-264-1640.

    Protocols will be sent to you for specific students and will be labeled with their names, and tracking codes.

    You may write in the protocols. Do no write in the Guidebooks.

    Instructions for Returns:

    All items are due back in the office within 45 days. You will be contacted 10 days prior to the return date as a reminder. Items can be returned to the main office in person, or send back in with your monthly billing.

    Fees and Penalties:
    Provider is responsible for cost associated with shipping the items back to the office. Provider is also responsible for any cost associated with electronic submission of assessment results.

    Provider is responsible for the full cost of each book, plus a $25.00 re-stocking fee (per book) if:
    The books are lost or stolen
    There is considerable damage rendering the book(s) unusable.
    The book(s) are not returned by the due dates.

    If the terms and conditions are not met, the provider will relinquish the privilege of participation in the assessment lending library.
