2017 Senior Award Nomination Form

Thank you for your nomination. The following information must accompany this form to ensure that all candidates are judged on an equal basis. It is the responsibility of the nominator to make certain that all of the nomination materials are received by the Awards Committee. Additional information (such as articles, letters of support, etc.) may accompany this form. A transcript is not necessary, but please confirm with the student that he/she has a GPA of 3.0 or higher. Incomplete nominations will not be considered by the Awards committee.

Required documents:
• Nominee's resume
• At least one letter of support from one of the nominee’s constituencies (administration, faculty, staff, alumni) stating the reasons for the nomination

The deadline for the 2018 Senior Awards will be announced Fall 2017.

Mark your calendars! This year’s reception and ceremony will take place on April 8, 2017.
