I hereby grant permission for the students listed above to participate in the After-School Sports Program at WISE Academy. I understand that all reasonable safety precautions will be taken; however, I acknowledge that the school cannot be held liable for unforeseen injuries or medical conditions that may arise during participation.

I agree to ensure that my child is promptly picked up following each practice session and/or game for the duration of the season.

By joining a WISE Academy athletic team, you are making a commitment to your teammates and to your school. Participation in this program requires dedication, responsibility, and respect both on and off the field. Please review the expectations carefully.
Attendance and Participation
Attendance at all practices and games is required.


An unexcused absence from a practice or game will result in being benched for part or all of the next game.


Classroom attendance is mandatory to participate in practices or games. Students who are absent from class may not participate that day unless the absence is prearranged and excused.


Injured athletes are expected to attend all practices and games to remain engaged with the team.


If less than 50% of the team attends three or more practices/games, the season may be canceled and refunds will not be issued.


Academic and Behavioral Eligibility
Students must be passing at least 66% of their classes to participate in games (e.g., 7 classes = passing 5; 6 classes = passing 4).


The Athletic Director (A.D.) will make all final decisions regarding academic or behavioral eligibility.


If an athlete quits a team without prior approval from the A.D., they will not be permitted to participate in any sports the following school year.
WISE Academy Athlete Standards
WISE Academy athletes are expected to:
Demonstrate integrity and display good sportsmanship at all times.


Always support and encourage their teammates.


Be coachable, open-minded, and willing to learn.


Accept their role on the team and work diligently if they wish to change it.


Take responsibility and avoid transferring blame or making excuses.


Serve as a positive example off the field, especially in the classroom.


Be punctual, prepared, and professional in all team activities.


Have fun, learn the game, and value the privilege of participation.
