I certify that all information provided in this application, including any attached documents, is complete and accurate. I acknowledge that providing false, fraudulent, or inconsistent information may result in, but is not limited to, disciplinary action, denial of exam admission, invalidation of earned credit, and forfeiture of any fees paid.

I understand that all fees must be paid by the specified registration deadline.

I understand that the the exam will not be ordered if payment is not received. I will make sure that the payment is submitted along with this form.

I also acknowledge that exam registrations will not be accepted beyond the deadline stated above.

I recognize that registration fees are non-refundable and that I cannot change my exam date once this application has been submitted. Note: Exam fees are subject to change without notice.

Upon submission of this application and the non-refundable exam fee, your application will be processed by our credentialing department. If approved, you will receive your exam permit directly from NBME via email.
