Purpose
GymCats and NinjaCats occasionally take photographs and short video clips of classes, events, and activities to celebrate students’ progress, recognize achievements, and communicate with our community through newsletters, websites, and social media. We respect that some families prefer not to have their child appear in any public media. This form allows you to request that your child not be intentionally included in such materials.
Parent/Guardian Request
I, the undersigned parent or legal guardian, do not consent to the use of my child’s name, image, or likeness in any GymCats / NinjaCats promotional or public media, including but not limited to photographs, video recordings, digital posts, or printed materials.
Acknowledgment of Limitations
GymCats and NinjaCats will make every reasonable effort to honor this request and will instruct staff accordingly. However, I understand and acknowledge that:
• The program operates in shared and public spaces where other parents, guests, or visitors may take photos or videos beyond the organization’s control.
• My child may appear incidentally in group photos, background shots, or live footage despite staff precautions.
• Complete exclusion from all public media cannot be guaranteed in such environments.
By signing below, I recognize that GymCats and NinjaCats will act in good faith to respect my wishes while operating within these real-world limitations.
