Payment Authorization Terms and Conditions:

I authorize DiMaggio Travels Inc. to charge the credit card indicated in this authorization form to the travel suppliers according to the terms outlined below. This payment authorization is for the goods/services described. Unless specified otherwise, all deposits are non-refundable. I understand that the accredited travel supplier that is affiliated with DiMaggio Travels will be charging my card for the amount or in combination to equal the above amount.

I certify that I am an authorized user of this credit card and that I will not dispute the payment with my credit card company; so long as the transaction corresponds to the terms indicated in this form.

I also reviewed all trip details and confirm that all information is correct and any alterations to the trip may result in extra fees.

I acknowledge that I am purchasing this trip using a travel agent and not with the airline/hotel directly therefore the transaction cannot be disputed due to flight/hotel service dissatisfaction.

I am also responsible for having any up to date passport/travel documents/visas etc. , and I am responsible for any consequences that may arise for not having them.

I understand that I am responsible to keep track of all payment due dates. Late final payments will result in forfeit of deposit or of a loss of this vacation.

In the event of a cancellation, for any reason, DiMaggio Travels Inc. reserves the right, to charge this card a $100 per person Cancel Fee for time and services provided.
