Certified information: Legal Names & birthdates, travel dates, flight times, airline, airline fees, insurance terms & conditions, car rental information, hotel supplier information, cancellation and/or change fees/policies, deposit/payment schedule.

I certify the above provided information to be true and correct. I have double checked all information is entered correctly. I understand that if a spelling is incorrect or date wrong, that I, and not Discovereighng with Dana Travel/Dana Lewis, am responsible for any change fees.

I certify that I understand that if I am traveling via air travel (airplane), that a valid passport (if traveling outside of the U.S.) or valid government issued photo ID are required for travel., and that for travelers under 18 with no valid ID, a official certified birth certificate is required.

I certify that I understand that a valid U.S. Passport is required for traveling outside of the United States, unless traveling on select cruises. I understand that it is my, the traveler's, responsibility to obtain a passport in ample time before travel. I understand that if my passport is not obtained in time, that I, the traveler, am personally responsible, and in the event that I am unable to travel, I understand that I may or may not receive any refund.

I certify that I understand that my passport must be valid for 6 months past my travel dates, or I may be denied entry into country I am traveling to.

I certify that I understand that if I am traveling on a cruise, that I must have a valid passport or official, certified birth certificate and, if over 18, a valid government issued photo ID.

I hereby verify that I have reviewed my travel itinerary for accuracy. I have been advised of all applicable fees imposed by Discovereighng with Dana Travel/Dana Lewis and the suppliers as well as any scheduled fees that may be required if this booking is cancelled or changed.

Discovereighng with Dana Travel/Dana Lewis offers retail travel services to customers, which are provided by separate and independent vendors of travel services. Discovereighng with Dana Travel/Dana Lewis does not operate, control, or otherwise provide the services of the independent travel vendors.

I agree and understand that Discovereighng with Dana Travel/Dana Lewis is not responsible for any operating failures, loss, damage, delay, or injury to me or members of my travelling party, nor for consequential damages, which may result from failure, delays, or injuries, which arise out of or during the scheduled travel. I agree and understand that Discovereighng with Dana Travel/Dana Lewis is not responsible for any damages, direct or consequential, which may arise as a result of acts of God, social or labor unrest, war, political or national strike, terrorist activities, mechanical failures, climatic conditions, or any other actions or omissions beyond the control of Discovereighng with Dana Travel/Dana Lewis.
