Before proceeding, please read through this form carefully, as it is a legally binding agreement.

Next, we will gather additional information from all travelers, collect payment for the deposit, and finalize your decision on travel insurance.

Each traveler responsible for their own deposit must complete this form to secure their reservation. All roommates must book on the same day. Please ensure that everyone in your travel party submits their form as well. Reservations will only be confirmed once all roommate forms have been received. For group bookings, you and your roommate may proceed as soon as you're ready; there is no need to wait for other group members to book.

Please note that submitting this form does not automatically apply your payment; I will process payments. Likewise, any credit card authorization forms you complete do not automatically charge your card.

  • International Trips Travel Document Requirements

  • International Air Travel:
    • Passport Book Required: A valid passport book is required for all international air travel. Passport cards are NOT valid for air travel.
    • Validity: Passports must be valid for at least six (6) months beyond the return date.
    • Blank Pages: Some countries require a minimum number of blank pages; it is recommended to have at least two (2) blank pages available.
    • Processing Time: Standard passport processing takes 4-6 weeks, with expedited services available. Travelers should allow sufficient time to obtain their passport before departure.

    Domestic Air Travel (Within the U.S.):
    • REAL ID Compliance: Beginning May 7, 2025, all travelers aged 18 and older will need a REAL ID-compliant driver’s license or another TSA-accepted form of identification to board domestic flights.
    • Alternative Acceptable IDs: Acceptable forms of ID include a valid U.S. passport, U.S. passport card, DHS trusted traveler cards (Global Entry, NEXUS, SENTRI, FAST), U.S. military ID, and other forms listed by TSA.

    Land and Sea Travel:
    • Canada & Mexico:
    • A passport book or passport card is required for land and sea entry.
    • Some states issue Enhanced Driver’s Licenses (EDLs) that may be used for land and sea travel.
    • Caribbean & Bermuda:
    • A passport book is required for air travel.
    • For land and sea travel, a passport card or EDL may be acceptable.
    • Cruises:
    • Closed-Loop Cruises (departing from and returning to the same U.S. port): U.S. citizens may present:
    • A government-issued photo ID AND
    • An original or certified copy of their birth certificate
    • Important Note: It is strongly recommended that all cruise passengers travel with a passport book in case an emergency requires air travel home.

    Minors Traveling Internationally with Only One Parent or Guardian:
    • Consent Letter: When a minor is traveling internationally with only one parent or guardian, it is strongly recommended to carry a notarized letter of consent from the non-traveling parent(s) or legal guardian(s). This letter should include:
    • The child’s full name and date of birth
    • The accompanying adult’s full name and relationship to the child
    • Details of the travel itinerary, including destinations and dates
    • Contact information for the non-accompanying parent(s) or guardian(s)
    • Purpose: This letter proves parental/guardian consent for international travel. While not always required, some countries and airlines may ask for it to prevent child abduction and custody disputes.
    • Notarization: It is highly recommended that the letter be notarized to ensure its acceptance by immigration authorities.
    • Exceptions:
    • If the accompanying parent has sole custody, they should carry legal custody documents.
    • If the other parent is deceased, a copy of the death certificate should be provided.

    Important Reminder: Travel requirements vary by country. It is the traveler’s responsibility to verify the latest travel document requirements for their destination before departure.
  • Travel Protection

  • Travel protection is not required, but is strongly recommended by Endless Destinations, LLC to help protect your travel investment. Travel protection may help cover expenses related to:

    - Trip cancellation or interruption
    - Airline delays, cancellations, or missed connections
    - Travel delays requiring additional hotel nights, meals, or transportation
    - Extended stays due to airline disruptions, weather, or medical situations
    - Emergency medical and transportation needs abroad
    - Emergency medical evacuation
    - Lost, stolen, or delayed baggage and personal belongings
    - Travel accidents and rental car physical damage

    When traveling internationally, U.S. medical insurance may not be accepted, and you may be responsible for emergency medical costs. Travel protection may help cover these unexpected expenses.

    If you choose to decline travel protection, you acknowledge that you assume full financial responsibility for costs related to cancellations, interruptions, delays, extended stays, medical emergencies, baggage loss, or other travel disruptions, and that Endless Destinations, LLC is not responsible for such costs.

    Please note that most travel protection premiums are non‑refundable once issued, even if you later cancel your trip or choose not to file a claim.
  • Terms & Conditions

    Please read through all of Endless Destinations, LLC Terms and Conditions thoroughly and carefully. If you have any questions or need clarification please contact me before proceeding.
  • By checking this box, you acknowledge that you have received and reviewed the travel supplier's itinerary in full, and confirm that all details, including travel dates, flight times, destinations, and the spelling of names, are accurate. Changes made after the booking has been made will incur a $50 per person change fee, as well as any airline-imposed or supplier fees.
  • I understand that cruise lines may adjust itineraries and airlines may make schedule changes. These changes are beyond the control of Endless Destinations LLC, and while every effort will be made to assist with adjustments, any resulting impacts are subject to the policies of the cruise line or airline.
  • By checking this box, you are acknowledging you have received the travel supplier's itinerary and coordinating booking documents that outline the travel supplier(s)'s terms and conditions from Endless Destinations, LLC. By checking this box, you are agreeing that you have read and reviewed the travel documents you have received. Some travel suppliers may reserve the right to change their policies.

    These Suppliers included but are not limited to:
    Walt Disney Travel Company, Royal Caribbean, Celebrity, Azamara, SilverSea, Carnival Cruise Line, Expedia TAAP, Apple Leisure Group Vacations, Universal Resorts, SeaWorld Parks and Entertainment, Apple Vacations, Blue Sky Tours, Funjet Vacations, Southwest Vacations, Travel Impressions, United Vacations, Delta Vacations, Allianz, AmStar, Vacation Express, Beds, Online, Globus Family of Brands, Classic Vacations, Avanti, and other preferred partners.
  • Effective March 26th, 2025, any changes and/or cancellations made to a booking once the deposit has been applied, including adding or removing travelers, changing travel dates, destinations, or accommodations, will incur a non-refundable fee of $50.00 per traveler.

    This fee compensates for the professional time dedicated to managing your booking and is separate from any additional cancellation or change fees imposed by travel suppliers. Please note that these fees are applicable regardless of the reason for the change or cancellation. Endless Destinations reserves the right to collect this fee per traveler before processing any change or cancellation requests.
  • Federal law forbids the carriage of certain hazardous materials, such as aerosols, fireworks, and flammable liquids, aboard the aircraft. If you do not understand these restrictions, contact your airline or go to http://www.faa.gov/about/initiatives/hazmat_safety.
  • By checking this box, I acknowledge that I have requested Endless Destinations, LLC to make one or more travel bookings on my behalf. I understand that travel plans may be affected, delayed, or canceled due to pandemics, public health events, political or civil unrest, or other unforeseen circumstances. I also understand that the supplier’s cancellation, rebooking, and refund policies, subject to applicable law, will determine my right to any refund. If I choose to purchase travel insurance, I understand that coverage is subject to the terms of the policy. I agree that Endless Destinations, LLC is not liable for any financial loss resulting from these circumstances.
  • Changes or cancellations made within 24 hours of departure are subject to the policies of the airline, hotel, or travel provider. Endless Destinations is not responsible for last-minute modifications, cancellations, or penalties imposed by these suppliers.

    If you need immediate assistance outside of regular business hours and I am unavailable by call or text, you must contact the During Travel Support Line provided in your travel documents. This ensures your request is handled as quickly as possible.

    Failure to show up for your flight, hotel, or other travel arrangements without prior cancellation will be considered a no-show. Travel insurance does not cover no-shows, and you may forfeit the full cost of your trip.
  • Endless Destinations will assign seats for clients on all major carriers, except Frontier. However, seat assignments are subject to change at the discretion of the airline due to schedule changes, equipment swaps, or other operational decisions. Endless Destinations is not responsible for any seat changes made by the airline after booking. Clients are encouraged to check their reservations regularly and contact the airline directly for any seat-related concerns.
    If you need immediate assistance outside of regular business hours and I am unavailable by call or text, you must contact the During Travel Support Line provided in your travel documents. This ensures your request is handled as quickly as possible.

  • I understand that Endless Destinations is applying the payment I am submitting to one or multiple of their accredited travel suppliers, such as Funjet Vacations, Apple Vacations, Travel Impressions, Disney, Delta Vacations, Vacation Express, Carnival Cruise, Royal Caribbean, GoGo Vacations, Southwest Vacations, Goway, GoElite, Pleasant Holidays, Classic Vacations, Norwegian Cruise Line, Rental Escapes, and/or Avanti Destinations. Therefore, your bank statement will have the charge posted from the supplier(s). Any payments that I submit towards this booking will be charged to the same supplier(s). Endless Destinations by Rachel will apply my payment to the supplier 24-48 hours after my submission of my payment.
  • If you have any criminal background, warrants, owe child support, owe taxes etc. and you're denied by the airline/cruise line/country because of background issues, Endless Destinations, LLC is not liable. Endless Destination, LLC is not liable for any refunds. Please check with your lawyer or government agency as it is your responsibility to know if you can travel or not.
