INFORMATION ABOUT THE NEUROPSYCHOLOGICAL EXAMINATION
LIMITS TO CONFIDENTIALITY
Although Dr. Kinsora will keep the information that you give us protected from those not involved in this case, the evaluation and all of the records generated by this examination are not covered under HIPAA and no doctor-patient relationship is present during the examination. If you have questions about this important matter, it is recommended that you speak with your attorney. The final results will be forwarded to the party who requested the assessment. If the evaluation is related to a workers compensation claim or disability claim, your results will go directly to the insurers involved in your claim. If the evaluation involves a lawsuit, attorneys on both sides of the case will typically have access to the results of this examination. You should be aware that if Dr. Kinsora is asked to testify in court, information that you share with us may be shared with the court in his testimony for the purposes of the legal proceedings. According to state law, under the following conditions, confidential information can be divulged without anyone's permission, whatsoever:
1. When disclosing the information “is necessary to protect against a clear and substantial risk of imminent serious harm by the patient to the patient or another person....”. NAC 641.224
2. When ordered to disclose information by a legal authority such as a judge or court administrator.
3. When required by law, such as when there is reasonable suspicion of abuse to a child or vulnerable adult.
4. If you are involved in litigation and the information obtained as part of the assessment is deemed to be a necessary part of the litigation, or when the assessment itself is part of the litigation procedure.
INFORMATION ABOUT THE TESTS
When you come in to see us you will be taking tests that are related to how well your brain processes information, perceives pain, and copes with emotional events. You may be asked to remember stories, draw, hold numbers in your mind, and respond rapidly to letters on a computer screen. We may test your ability to solve conceptual problems, and to answer questions about the world. On some tasks you may need to be as fast and accurate as you can, and on others you are only required to be as accurate as possible. We may measure finger-tapping speed, fine motor skills, and grip strength. May test your sense of smell. We may ask about anxiety, depression and other emotional symptoms that you have. We will need to know about past traumas that you have experienced in your life Most test take between five and 15 minutes to complete, but there are a lot of them to complete during the day that you are with us.

Foreseeable Risks, Discomforts, and Benefits:
The vast majority of examinees find the assessment challenging, and intriguing. Some call the test that we administer "fun". Either way the tests can be challenging, and you will need to really use your brain power to do well on them. Rarely, neuropsychological examinations can cause temporary fatigue, frustration, and anxiety. We will try to make this an enjoyable experience for you.
Breaks and lunch: Breaks are permitted between each subtest, as needed, and we will rely on you to let us know when a break is needed, but will also take them intermittently as necessary. Again, most test take between five and 15 minutes to complete. Be sure to pack a lunch and some snacks!
ILLNESS: If you wake up on the day of the examination and are clearly ill, or you know that you are ill in the days before the exam, please do not come in. Call your attorney immediately and let them know that you are ill. We will reschedule you as soon as we can over the coming weeks.
