To help the clinical interview go smoothly and efficiently, we need you to complete the attached intake form. This will save time and reduce the time that you will need to be in the office.

Please be sure to complete the entire intake form in detail; this will save you time having to go over it in person and will shorten your time in our office.

Finally, please be sure to refrained from using any type of alcohol, cannabis products (even if prescribed), or recreational substances other than your non-cannabis prescription medications for five days before the assessment. Do not complete this form if you are intoxicated now.

You will be with is for at least 8 hours, although some evaluations are spread over two days and last about 12 hours.

Try to get as much sleep as possible the night before the appointment.

Bring a lunch, snacks, and something to drink (we can provide water). You will be provided breaks and a break for lunch.

You are welcome to take assistive devices, such as reading glasses, canes, walkers, lumbar support pillows, and most other common products to keep you comfortable and safe.

Take the medications that you are normally prescribed on the day of the evaluation (except the cannabis products). Bring any medications that you will need to take later in the day. PLEASE TAKE NO MORE THAN THE PRESCRIBED AMOUNT OF ANY PAIN MEDICATION OR MUSCLE RELAXER.

    The information that you enter on this intake form goes to a secure server where the information is stored. The server is fully HIPAA certified and complies with the technical safeguard section of the HIPAA security rule. For example, the server has passed Level 2 PCI certification, which is high level security requirement necessary to protect private information. The servers SSL certificates support high-grade 256-bit encryption. This online intake is being offered as a convenient option to the hand written form.   • INFORMATION ABOUT THE NEUROPSYCHOLOGICAL EXAMINATION

    LIMITS TO CONFIDENTIALITY

    Although Dr. Kinsora will keep the information that you give us protected from those not involved in this case, the evaluation and all of the records generated by this examination are not covered under HIPAA and no doctor-patient relationship is present during the examination. If you have questions about this important matter, it is recommended that you speak with your attorney. The final results will be forwarded to the party who requested the assessment. If the evaluation is related to a workers compensation claim or disability claim, your results will go directly to the insurers involved in your claim. If the evaluation involves a lawsuit, attorneys on both sides of the case will typically have access to the results of this examination. You should be aware that if Dr. Kinsora is asked to testify in court, information that you share with us may be shared with the court in his testimony for the purposes of the legal proceedings. According to state law, under the following conditions, confidential information can be divulged without anyone's permission, whatsoever:
    1. When disclosing the information “is necessary to protect against a clear and substantial risk of imminent serious harm by the patient to the patient or another person....”. NAC 641.224
    2. When ordered to disclose information by a legal authority such as a judge or court administrator.
    3. When required by law, such as when there is reasonable suspicion of abuse to a child or vulnerable adult.
    4. If you are involved in litigation and the information obtained as part of the assessment is deemed to be a necessary part of the litigation, or when the assessment itself is part of the litigation procedure.

    INFORMATION ABOUT THE TESTS

    When you come in to see us you will be taking tests that are related to how well your brain processes information, perceives pain, and copes with emotional events. You may be asked to remember stories, draw, hold numbers in your mind, and respond rapidly to letters on a computer screen. We may test your ability to solve conceptual problems, and to answer questions about the world. On some tasks you may need to be as fast and accurate as you can, and on others you are only required to be as accurate as possible. We may measure finger-tapping speed, fine motor skills, and grip strength. May test your sense of smell. We may ask about anxiety, depression and other emotional symptoms that you have. We will need to know about past traumas that you have experienced in your life Most test take between five and 15 minutes to complete, but there are a lot of them to complete during the day that you are with us.

    Foreseeable Risks, Discomforts, and Benefits:

    The vast majority of examinees find the assessment challenging, and intriguing. Some call the test that we administer "fun". Either way the tests can be challenging, and you will need to really use your brain power to do well on them. Rarely, neuropsychological examinations can cause temporary fatigue, frustration, and anxiety. We will try to make this an enjoyable experience for you.

    Breaks and lunch: Breaks are permitted between each subtest, as needed, and we will rely on you to let us know when a break is needed, but will also take them intermittently as necessary. Again, most test take between five and 15 minutes to complete. Be sure to pack a lunch and some snacks!

    ILLNESS: If you wake up on the day of the examination and are clearly ill, or you know that you are ill in the days before the exam, please do not come in. Call your attorney immediately and let them know that you are ill. We will reschedule you as soon as we can over the coming weeks. You are welcome to leave a message at 702 382-3670, even if it is before opening hours. Just leave some details, including your name and a callback number.
  • VERY IMPORTANT GROUND RULES THAT YOU MUST AGREE TO

    Below are some important rules that we have for you to follow.

    Rule 1: Be honest and forthright when answering the questions below and try your best on all of the tests when you come in for the examination. Some of the tests are very hard and some start easy but get harder and harder as they progress, it is perfectly normal to struggle on many of the tests. However, it is important that you do not give up. Please tell us if any issue comes up during the day that might interfere with you doing your best on the assessment (i.e. severe pain, headache, fatigue, etc.). If you believe that you are having an unusually bad day and cannot perform at your optimal level (given what ever medical conditions you have), we can stop and reschedule you to finish on another day. This exam is meant to measure how well you function on a normal/typical day given your current condition, not how you are doing on an exceptionally bad day. We would like to make the day that you spend with us as comfortable for you as we can.

    Rule 2: RECORDING AND TAKING NOTES DURING THE ADMINISTRATION OF STANDARDIZED TESTS ARE STRICTLY FORBIDDEN. We are required by law to protect the security of the tests that we administer (Nevada Administrative Code 641.234). In an effort to keep our legal and ethical obligations, and preserve the validity of the measures, we strictly forbid any type of recording (audio, video, photography, live feed, across cellular or other transmission device, and others), direct or indirect observation, and any type of note-taking during the assessment day.

    Rule 3: Absolutely no one other than the examinee, Dr. Kinsora, and Dr. Kinsora's staff members are allowed in the room during the evaluation.

    Rule 4: If mutually agreed upon at least one week prior to the examination, Dr. Kinsora will allow the audio recording of the clinical interview portion of the evaluation only. Absolutely no recording of during test administration will be allowed. However, both parties agree to use only one identified recording device each, and that the device be presented as a recording device to Dr. Kinsora and placed in an open area on the desk between the examinee and Dr. Kinsora.

    Rule 5: No monitoring device will be used to remotely monitor or record any portion of the clinical interview or evaluation today.
