High Five Nomination Form
This is the nomination form for the High Five recognition award. The form must be filled out in its entirety / all fields must be completed in order for those nominated to be considered and approved.

Upon approval from the appropriate supervisor and HR Representative, the recipient will receive a personalized letter and recognition certificate signed by the functional Vice President (or leader), 5,000 Kazoo employee recognition points redeemable for great rewards, and recognition for the award through announcements via Dal-Tile Business Communications channels (including email, The Scoop magazine and digital e-signs in Dal-Tile facilities).
  • Criteria:

    • The Dal-Tile High Five program is open to all Dal-Tile U.S. (including Dallas IS) and Canada team members.

    • Team members may nominate the work of another team member for recognition (i.e. peers, co-workers, internal clients, leaders, direct reports, etc.)

    • A nominated team member must demonstrate action or work that goes “above and beyond” their expected performance level and the action or work must provide a quantifiable benefit to other team members, customers and/or the Company.

    • The nominated team member must be in compliance with all Company policies, in good standing and must meet all requirements and expectations for their position.
