2026 US Bocce Nationals Master Registration Form
* As the captain or group leader, use this form to enter the number of each events you are registering and paying for at the bottom of the form. Payments will ONLY be made with this form. Payments for FULL teams is only accepted.

* Be certain to also fill out the individual registration forms for each tournament that includes team name, captain, players' names and shirt sizes.

* Tournament Registration Forms are available at www.usbf.us under the Nationals tab.

* The amount you'll be paying by credit card will be calculated automatically on this form.

* Fill out the team names and precision shooting players to be listed in the tournament brackets.

* Add the names of the Awards Banquet attendees here as well.
