In consideration of $0 Dollars ($0) receipt of which is acknowledged, I/We, the undersigned, do hereby give Wartburg College, his or her assigns, licensees, and legal representatives the irrevocable right to use my child’s name (or any fictional name), picture, portrait, or photograph in all forms and media and in all manners, including composite or distorted representations, for advertising, trade, or any other lawful purposes, and I waive any right to inspect or approve the finished version(s), including written copy that may be created in connection therewith. I have read this release and am fully familiar with its contents.

My child has permission to participate in the event selected above.
If illness or injury may occur during the camp, I give permission to authorized personnel to seek medical treatment for my child. The parent/guardian will be responsible for all financial obligations associated with the medical treatment. I realize that camp/festival personnel will make every effort to contact the parent/guardian first.

I/We, the undersigned, for ourselves, our heirs, executors and administrators, waive, release and forever discharge Wartburg College and its staff, officers, agents, employees, representatives, successors and assigns from any and all liability, claims, demands, actions, and causes of actions whatsoever arising out of or related to any loss, personal injury, or property damage that may be sustained or occur during participation in camp activities or while at the camp/festival.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious. Wartburg College follows CDC guidelines. Vaccines are readily available. Masks are welcome but not required regardless of vaccination status. Participants will adhere to additional mitigation guidelines such as practicing good hand hygiene. Mitigation policies and protocols are subject to change based on current guidelines and recommendations.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and my child(ren) may be exposed to or infected by COVID-19 by participating in a Wartburg camp/festival and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at a Wartburg camp may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Wartburg employees and other campers and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or my child(ren) (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense of any kind, that I or my child(ren) may experience or incur in connection with my participation or my child(ren)’s participation at a Wartburg camp/festival (“Claims”). On my behalf, and on behalf of my child(ren), I hereby release, covenant not to sue, discharge, and hold harmless Wartburg, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Wartburg, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Wartburg camp/festival.
