How to Request your Clearance Card Renewal Electronically: 


Step 1: Submit completed copy of this registration form 


Step 2: Once you have submitted the form, Campus Health Centre staff will verify your information


Step 3:Process your payment ($35.00, MasterCard, VISA): Funds will be held until your information has been verified


Step 4: Electronic Clearance Card will be sent to the email address used in this form
