Candidate Information Form 2024
The Candidate Information Form provides information about you and your qualifications for higher office to members. This information, along with your one-page flyer, will be included in a Candidate Information Booklet that will be distributed to all attendees at the District Educational Conference.
This form must be electronically submitted by March 1, 2024. In addition, your service agreement and candidacy forms must be mailed or emailed to the District Administrator by March 1, 2024 to be an official candidate at the District Educational Conference.
You will receive an email receipt upon successful submission of this form.
