Prior to the adoption of a dog, we ask that you complete this application. This information will help HDPS achieve its goal of finding permanent, responsible, loving homes for the animals in our care and allow HDPS to better assist you in finding a pet suited to your needs.

The adoption fee is $200.00. Personal checks or cash will be accepted. Your adoption fee is considered a charitable, tax deductible donation. All adult dogs are fully vetted, spayed/neutered, heartworm negative/treated.

In order to be considered as an adopter you must:
* Be at least 21 years of age.
* Have the knowledge and consent of your landlord, if renting.
* Be able and willing to spend time and money necessary to provide training, medical treatment and proper care for a pet.
